HOW TO COMPILE AN APPLICATION FILE
Where to obtain the MDPH application form?
- It can be found at the Departmental House for Disabled Persons (MDPH) or the House of Autonomy (MDA) (an information, reception, and assessment service for the elderly and disabled persons)

How to submit the application?
It can be sent by registered mail: make a photocopy of the registered mail receipt in case it is lost

The MDPH application process
1. Submission
The application must be sent by registered mail with acknowledgment of receipt, or submitted in person to the MDPH reception, with an acknowledgment of receipt issued.
2. Registration
If the application is complete, an acknowledgment of receipt will be issued. Otherwise, a letter will be sent to you indicating a deadline for submitting the missing documents
3. Assessment
The application is assessed by a team of professionals. You have the right to request to be present during this multidisciplinary assessment
4. Proposal
The EPE proposes a personalized compensation plan (PCP) and, if the child is enrolled in school, a personalized schooling plan. These are sent to you by mail, and you have 15 days to accept or refuse the proposals
5. Decision
The CDAPH meets to rule on the applications. If the simplified procedure box has not been checked on the application CERFA form, you must receive a summons at least 15 days before the commission date
6. Notifications
Notifications are sent and transmitted to partner organizations. As a precaution, it is also advisable to send a copy of the notification by registered mail with acknowledgment of receipt (LRAR) to the CAF or the academy you depend on
THE MEDICAL CERTIFICATE
The medical certificate is a mandatory component for compiling the application file
To complete it: prefer the referring specialist physician during a consultation dedicated solely to the medical certificate
It can also be completed by your general practitioner who is familiar with your illness
Supplement it with medical reports (and not your blood tests or X-rays)

The multidisciplinary team
of the MDPH
The team meets to assess your application:
– It is composed of medical and paramedical professionals with diverse areas of expertise: educational, social, university employment, vocational training…
– The team gathers to assess disability compensation needs according to the nature and type of disability
– It is based on the administrative file, the life project, and the medical certificate
The PCP: personalized compensation plan
Once the multidisciplinary team has met, it proposes the PCP.
The PCP includes proposals for measures of all kinds, particularly concerning rights or benefits mentioned in Article L.241-6, intended to provide, to the disabled person, in light of their life project, compensation for the activity limitations or restrictions on participation in social life they encounter due to their disability.
The compensation plan is transmitted to the disabled person or, where applicable, to their legal representative, who has a period of fifteen days to submit their observations. The Commission for the Rights and Autonomy of Disabled Persons is informed of these observations.
To contest, it is necessary to add and supplement the information already provided.
It is possible to be assisted by a social worker.
The CDAPH
(Commission for the Rights and Autonomy of Disabled Persons)
This is the final stage of the application process; once the PCP is proposed, it is validated by the CDAPH, and the notification will be sent to you.
The CDAPH is composed of:
- Representatives of the Department
- State public services and institutions
- Social protection organizations (CPAM, CAF,…)
- Trade unions
- Parents’ associations
- At least one-third of its members are representatives of disabled persons and their families, designated by representative associations (you may attend the commission meeting or contact the representative association)
- A member of the Departmental Consultative Council for Disabled Persons
- Representatives of organizations managing institutions or services sit on the commission in an advisory capacity.
