Day dedicated to the transition to adulthood for adolescents with rare diseases: TOULOUSE

This information day aims to prepare young patients to:

– transition to adult care services without stress,

– gain independence through appropriate resources.

The program will include (see attached poster):

– a presentation of local stakeholders,

– a round table discussion on the various aspects of adolescent-to-adult transition,

– workshops for young people and parents,

– sports, music, and relaxation activities… and most importantly, many opportunities for discussion!

The day is free, but online registration is mandatory. ⬇️
